1Motor Vehicle Record (MVR) — Texas DPS and any other state I've held a license in the last 3 years (FMCSA 391.23 + 391.25)
2FMCSA Drug & Alcohol Clearinghouse query — pre-employment full query + annual follow-up (FMCSA 382.701)
3Contact with DOT-regulated previous employers — last 3 years of safety performance history including drug/alcohol test results (FMCSA 391.23 + 40.25). Previous employers are released from liability for providing this information.
4DOT pre-employment drug test (49 CFR 40 + 382.301) as condition of employment. Upon hire: ongoing random pool, post-accident, reasonable suspicion, and return-to-duty testing per FMCSA 382.
5FMCSA Pre-Employment Screening Program (PSP) report — 5 years of crash data + 3 years of inspection data.
6I certify all information in this application is true, complete, and correct. I understand false statements may result in rejection, termination if discovered after hire, and possible criminal prosecution under 49 CFR 390.35.
